Surgical Consents

In general, MaineHealth has transitioned to e-Consents for all procedures and operations.

If you are unable to file an e-Consent for whatever reason, paper consents are still available. You need some type of documentation that you had an informed consent conversation with the patient and/or their representative and that they have consented to the procedure. If you have to do a paper consent, be sure to put a patient sticker at the top of each page (or fill out their information) and try to upload a photo to the patient's media tab.
